Forms

In order to reduce your wait time to see one of the providers, we encourage you to fill out our forms.  As you know, paperwork is always the longest part of any visit to any new healthcare facility, and we would like to make your visit as smooth as possible, allowing us to focus on YOU, not your paperwork.

When you schedule your first appointment, we will email and/or text you a link where you can fill in these forms online. Simply go through and when you’re done, submit them. There’s no need to print them out, no need to bring them in. This saves us both time, and enables us to get you in for treatment when you’re in.

We are excited that you would consider honoring us with being your partners in health!   At the Natural Health & Pain Relief Center, we offer a “pateint-centered evidence based” approach to health, healing, and pain managment.  What this means is that everything that we do, from answering the phone, to greeting you at the door if you are a new patient, is all about your experience.  If you feel better and are welcomed warmly, then you will start the healing process as soon as you start feeling relaxed and loved.

Your typical new patient visit may last anywhere between 40 minutes and 90 minutes, depending on the provider you see.  We will typically welcome you at the door and escort you to a private room to fill out paperwork while offering you some water.

If you don’t know “what you need” to get better, then you will most likely see Dr. Wellwood who acts as our resident “gatekeeper”. Because of his advanced knowledge, he may be able to refer you to our acupuncturist, occupational therapist, massage therapist, functional medicine, or to begin chiropractic work.  As an integrative facility, you will most likely see one or more of our providers concurrently as this offers the best way to get out of pain quickly and efficiently, while reducing the amount of treatment duration you may otherwise need.

What Can I Expect?

Going to the Integrative Healthcare Facility is a new experience for many of us. Maybe we’ve heard through a friend or have done some research online. It’s possible that you are just fed up with living in pain and it’s time to do something about it. In either case, your first visit to the chiropractor will really be about getting to know the chiropractor and discussing your history, current condition and goals.

As with any doctor’s visit, you’ll start by finishing some new patient paperwork. Upon meeting with your chosen healthcare provider, you’ll discuss your concerns and expectations as well as have the chance to ask any questions you may have. We will perform a physical examination and most likely perform additional advanced testing procedures, such as a surface EMG to see how your muscles are working, an infrared paraspinal thermograph to see if you have any heat/inflammation along the spine, a “dual inclinometer” to determine how well you are moving, or even an “acugraph” to see if acupuncture would be of value to your healthcare progreess. Once this is complete, we’ll take a look at the overall results of our conversation and the exams. Together, we’ll come up with a plan of action which will begin when you are completely satisfied and ready to move forward.
